Payment Options

We accept the following as payment for services rendered. Not sure what you qualify for? We’ll help you figure it out.

Medical Assistance (MA)

Health insurance for low-income people of any age. Apply through your local county human services department. For help, call the Senior LinkAge Line at 1-800-333-2433 or the Disability Hub MN line at 1-866-333-2466.

Waivered HCBS Programs (245D)

Minnesota Home & Community-Based Services waivers cover care for people who qualify. See the waiver programs below to learn which one may fit.

Private Pay / Private Insurance

Most private health and long-term care insurance plans include a home health care benefit — contact your carrier to learn your benefits. You can also pay privately for services not covered by other payers.

Alternative Care ProgramHome & community services for Minnesotans 65+ at risk of nursing-home placement, with low income and assets. Elderly Waiver (EW)For adults over 65 who need the level of care provided in a skilled nursing facility. Community Access for Disability Inclusion (CADI)For adults 18–64 with disabilities who require a skilled-nursing level of care. Brain Injury (BI) WaiverFor people with a traumatic or acquired brain injury needing nursing-facility or neurobehavioral care. Community Alternative Care (CAC)For chronically ill or medically fragile people who need a hospital level of care. Developmental Disabilities (DD) WaiverFor people with developmental disabilities or related conditions needing ICF-level care.
